Medical record release forms may be obtained to secure relevant records from the hospital(s), clinic(s), and/or laboratories where the proband has been evaluated. Relevant research data from external studies that participants have enrolled in may also be requested for use in this study. Requested medical and research record types include, but are not limited to, clinic notes, test results, genetic results, EEG results, copies of imaging scans/results, medications, pathology reports, and other relevant clinical and research data.
